Laparoscopic Diagnosis and Treatment of Meckel's Diverticulum Complicated by Gastrointestinal Bleeding

Abstract: Laparoscopy has been utilized for many gynecological and surgical procedures that previously required laparotomy. Its use in the diagnosis and treatment of Meckel's diverticulum complicated by gastrointestinal bleeding is described.

“…[5][6][7][8][9][10][11] The minimally invasive nature of CL in contrast to exploratory laparotomy appears a lot more appealing as a surgical option particularly when all the preoperative investigations are negative. Most of the previous reports described a 3-port technique to locate the pathology followed by exteriorizing the bowel segment bearing the abnormality for extracorporeal resection through the umbilical wound, which may or may not need to be extended.…”

[5][6][7][9][10][11] Others have reported laparoscopic intracorporeal diverticulectomy using Endo GIA stapler across the base of Meckel's diverticulum. 6,15,16 Such an approach in general is not advocated in those cases of Meckel's diverticulum complicated with bleeding, as it may not be adequate to remove all the ectopic gastric mucosa, and segmental resection of intestine bearing the Meckel's diverticulum is the preferred method 7,9,17 in such circumstances.…”

“…15 our cases, some researchers have recommended diagnostic laparoscopy ahead of bidirectional endoscopy in the evaluation of LGIB, as air insufflation during the procedure might hinder a successful laparoscopic evaluation under the same general anesthesia. 15 Resection of bleeding Meckel's diverticulum has been accomplished by laparoscopy-assisted extracorporeal bowel resection [14][15][16][17][18] or intracorporial suturing and stapling techniques. 19 However, there are anecdotal reports of the complete laparoscopic treatment of alimentary duplications 20 in children in the literature, so far.…”
