Laparoscopic hemicolectomy in a patient with situs inversus totalis

Abstract: As among persons with normal anatomy, occasional p a t i e n t s w i t h s i t u s i nv e r s u s d e v e l o p m a l i g n a n t tumors. Recently, several laparoscopic operations have been reported in patients with situs inversus. We d e s c r i b e l a p a r o s c o p i c h e m i c o l e c t o m y w i t h radical lymphadenectomy in such a patient. Careful consideration of the mirror-image anatomy permitted safe operation using techniques not otherwise differing from those in ordinary cases. Thus, curative la… Show more

“…The mirror image of the laparoscopic view creates an unfamiliarity for surgeons and their usual maneuvers. In addition, all of the instrument designs are for right-handed surgeons [3]. We present a case of a transperitoneoscopic nephroureterectomy involving a renal pelvis urothelial carcinoma patient with SIT.…”

“…Laparoscopy can prove to be an even greater technical challenge. Although laparoscopic colorectal surgery has recently become a standard procedure, there are only seven reports on laparoscopic surgery in patients with SIT [2][3][4][5][6][7][8]. There are three sigmoidectomies, two hemicolectomies, one abdominoperineal resection, and one total mesenteric excision.…”

“…Accordingly, surgical procedures are considered more difficult in patients with SIT than others because of different anatomic positions of organs, especially in laparoscopic surgery. While laparoscopic colorectal surgery recently has become a standard procedure, only seven previous reports of laparoscopic surgery for colonic disease in patients with SIT are available [2][3][4][5][6][7][8].…”

“…As in the case described, hemodynamically unstable patient had surgical indication without further investigation or examination 6,10,14 and no delay in trying to resuscitate the patient before the operation 4,9,15 . Due to the complexity of vascular anomalies, situs inversus in the past was considered a contraindication to liver operations.…”
