Laparoscopic Treatment of Gallstone Ileus

Abstract: Gallstone ileus is a rare complication of cholelithiasis that has high morbidity and mortality. An intestinal obstruction can be caused by migration of a large gallstone through a biliary enteric fistula or by impaction within the intestinal tract. In this study, we present the case of an 81-year-old woman with a mechanical bowel obstruction by a gallstone that was treated by laparoscopy.

“…The most common sites of impaction are represented by the terminal ileum, the ileocaecal valve, and more rarely the jejunum, duodenum, stomach, and in 4% of patients, the colon [2] . Clinically, it can present with the triad of Rigler [2] , [3] , [4] , [5] : pneumobilia, dilated small bowel with paucity of air in the large bowel, and opacity in the right iliac fossa.…”

“…The surgical approach is still variable depending on the characteristics of the patient and the underlying disease, and based on the judgment of the surgeon. When possible, the laparoscopic approach is feasible [4] . Our patient needed laparotomy due to the complexity of the disease.…”

“…Extracorporeal shock wave lithotripsy (ESWL) for stone fragmentation in gallstone ileus is rarely used because of its highly costs (20). Although laparoscopy has limitations in a situation of a gallstone ileus and conversion to laparotomy is frequent, experienced surgeons can perform enterolithotomy or disimpaction of the gallstone and address to biliary pole at the same time (21)(22)(23).…”

“… 17 Laparoscopic procedures can be an alternative method, although a surgeon with specialist experience in advanced laparoscopic surgery would be necessary. 18 19 20 The endoscopic treatment of gallstone ileus is a valid alternative approach and some cases have already been reported. 21 22 Ultrasound-guided extracorporeal shock wave lithotripsy (ESWL) has also been suggested as a non-invasive alternative to surgery to fragment the stone and solve the sub-occlusion.…”
