Laparoscopic treatment of renal hydatid cyst

Abstract: A 30-year-old woman was treated successfully for renal hydatid cyst disease by using the Transperitoneal Laparoscopic Technique. The peritoneal cavity was protected with the use of betadine-soaked gauze pieces, to avoid spillage. Hypertonic saline was used as the scolicidal solution to sterilize the cyst. The endocyst was removed completely and retrieved in an endobag. There were no intraoperative or early postoperative complications. This appears to be only the second reported case of renal hydatid cyst disea… Show more

“…To our knowledge, this is only the third reported case of laparoscopic transperitoneal partial nephrectomy for renal hydatid cystic disease, the first being reported by Basiri et al 4 and the second by Kumar et al 5 Other approaches to this rare disease entity have been described in the literature, including laparoscopic transperitoneal nephrectomy, 6 laparoscopic transperitoneal cystotomy, 5,7,8 and retroperitoneal laparoscopy. 9,10 There are important differences and similarities between our surgical technique and those of the previous 2 authors who performed laparoscopic transperitoneal partial nephrectomy mentioned above.…”

“…12 Some authors report a retroperitoneal approach, citing fear of contamination of the abdominal cavity in case of spillage. 8,9 However, we prefer a transperitoneal approach, as it allows us to gain better access to the renal hilum, in the event that a total nephrectomy has to be undertaken.…”

“…Parenchymal saving surgery remains the mainstay of treatment of renal hydatid cyst [15]. It may range from open surgery to laparoscopy and least invasive PAIR (percutaneous aspiration injection and respiration) technique [16].…”

“…During kidney-sparing surgery, scolicidal solutions such as hypertonic, cetrimide, povidone iodine should be used before opening the cavities to kill the daughter cysts and therefore prevent further spread or anaphylactic reaction. [25] Surgical management consists of various options. Total excision consists of either wedge resection or partial nephrectomy and partial excision includes partial pericystectomy followed by capitonnage.…”
