Large cervical osteophyte—another cause of difficult flexible fibreoptic intubation

Abstract: SummaryA patient sufering from ankylosing spondylitis required surgical excision of a large anterior osteophyte of the cervical spine. Fibreoptic nasal intubation was d3cult due to distortion of the airway by the osteophyte. This cause of dificultflexibleJibreoptic intubation has not been described previously.

“…Successful management has been reported with direct laryngoscopy, 1 an intubating laryngeal mask airway, 2 oral fiberoptic intubation, retrograde wire-guided fiberoptic intubation, 3 as well as tracheostomy. Limited neck and jaw mobility, anterior vertebral osteophytes resulting in airway distortion, 4 and cervical vertebral fractures from forced extension during intubation resulting in quadriplegia have been described. 5 Although it may be preferable to avoid instrumentation of the nasal mucosa in pregnancy, fiberoptic intubation via the nasal route may prove easier than via the oral route.…”

“…Bu hastalarda genellikle önerilen entübasyon şeklinin uyanık entübasyon olduğu görülmektedir. 7 Biz hasta reddettiği için uyanık entübasyon yapmadık, ancak bu anlamda şanslıydık ve ciddi havayolu problemi yaşamadık.…”

“…Sir:Tracheal intubation may be difficult in patients with cervical osteophytes [1–3]. We suggest that, in these patients, the laryngeal function may be impaired and cricoid pressure may be ineffective.…”
