Large oral focal mucinosis: a case report

Abstract: The aim of this article is to report the case of a 23-year-old female patient that sought dental service for examination of an asymptomatic slow-growing large lump in the mandibular gingiva causing displacement of the right first molar. Excisional biopsy, histopathological analysis, Alcian blue/periodic acid-Schiff (PAS) staining and immunohistochemistry analysis for S-100 were performed. The diagnosis established was oral focal mucinosis. After three years, the patient is still under follow-up, without any re… Show more

“…OFM stains positive with alcian blue, indicating the abundant mucins scattered around the connective tissue (3,10,15,17). Its treatment involves complete surgical excision, and no recurrence is reported (10,16,18). In the present case report, the excisional biopsy was performed.…”

“…In the reviewed articles, no changes in bony tissues had been reported, except for a bone displacement case [6] and a case of cervical resorption of the dental root adjacent to the lesion [18]. Excessive production of hyaluronic acid and reduced production of collagen are two important etiologies of OFM.…”

“…Clinically, this lesion appears as an asymptomatic, exophytic, sessile or pedunculated lesion containing a mucoid myxomatous substance surrounded by a relatively dense collagenous connective tissue [1,2]. The lesion is often similar to the normal oral mucosa in color [2] and is typically 1 cm or less in diameter [6]. The lesion appears to develop more frequently in the mucosa covering bony areas [4,7].…”

“…The myxoid tissue consist of an area of loose connective tissue formed by an accumulation of hyaluronic acid and widely spaced collagen fibers interspersed with stellate-shaped fibroblasts, with or without inflammation. [ 5 ] As similar features may be appreciated in odontogenic myxoma (OM) and nerve sheath myxoma, these lesions should be included in histological differential diagnosis of OFM. [ 6 ] OM is a true neoplasm of mesenchymal origin.…”
