Laryngeal Mask Ventilation during Neonatal Resuscitation: A Case Series

Abstract: Positive pressure ventilation via a facemask is a critical step in neonatal resuscitation but may be a difficult skill for frontline providers or trainees to master. A laryngeal mask is an alternative to endotracheal intubation for some newborns who require an advanced airway. We present the first case series in the United States in which a laryngeal mask was successfully utilized during resuscitation of newborns greater than or equal to 34 weeks’ gestation following an interdisciplinary quality improvement co… Show more

“…In contrast to neonatal intubation, LM insertion is a low-frequency emergency procedure with easier skill acquisition and maintenance than endotracheal intubation 21,22. LM insertion has been successfully taught to diverse healthcare providers using manikin-based training sessions 6,17,20,22,30. For providers who perform low-volume procedures infrequently, such as LM insertion, and have infrequent or no opportunity for proctored supervision, simulation may be the only feasible method for achieving the Do and Maintain steps of this training program.…”

“…On the basis of this information, OCH (level IV NICU), in collaboration with a level II nursery staffed by OCH providers, implemented a quality improvement initiative to increase availability of LMs, improve LM training at NRP provider courses, and "promote awareness of LM as a safe and effective alternative airway for neonatal resuscitation." 30 A retrospective medical record review revealed 10 cases of LM insertion by nurses, nurse practitioners, and physicians over a 15-month period. 30 There were 4 cases in each hospital's delivery room, one case during neonatal transport, and one case in the NICU.…”
