Laser-Doppler Examination Shows High Flow in Some Common Telangiectasias of the Lower Limb

Abstract: We interpret the higher flow values as a consequence of open AV shunts. This means that AV shunt pathophysiology was present in some of our cases.

“…Blood then will percolate through the system of this reticular vein plaque, being collected at its other side and channeled into a larger branch now in the common anterograde direction of the superficial network. The chronically high flow 13 induces then the pathologic remodeling of the reticular venous mass 1 . Reticular vein masses can be fed either directly by an incompetent perforans ( Fig.…”

“…In the first place, a backflow of the sclerosant into the arterial part of circulation either through arterio-venous microshunts or directly into arterial capillaries due to a large volume of or fast injection of the sclerosant. Bihari et al 1,8 published two studies detecting more than 60% incidence of AV microshunts in patients with telangiectasia. Although an attractive hypothesis, it is hard to reconcile an apparently low incidence of cutaneous necroses and so common presence of the supposed AV microshunts.…”

“…When a biopsy is performed in the presence of a pulsatile Doppler flow next to the telangiectasias, as many as 16 arterio-venous shunts/18 biopsies can be found. 33 In these cases, Bihari et al 34 found not only the feeding arteriole but also an arterialization of the draining vein and venules. They also observed a significantly higher flow in telangiectasias in comparison to the surrounding skin (5 areas of telangiectasias/22 legs).…”
