Learning curve of full-endoscopic lumbar discectomy

Abstract: Purpose To report the learning curve of full-endoscopic lumbar discectomy for a surgeon naive to endoscopic surgery but trained in open microdiscectomy. Methods From July 2006 to July 2009, 57 patients underwent full-endoscopic lumbar discectomy and 66 underwent open microdiscectomy. The clinical results were evaluated with a visual analog scale (VAS) and the Oswestry Disability Index (ODI). Spearman's coefficient of rank correlation (rho) was used to assess the learning curves for the transforaminal and inter… Show more

“…In this series, L5/S1 disc access was facilitated by careful patient positioning using a 'broken' operating table and no patient required intra-operative conversion to Micro. Transforaminal endoscopic surgery is associated with a significant learning curve [28]. All five revisions in the TED arm of the trial were within the first two-thirds of the study.…”

“…These approaches are associated with less soft tissue and bony trauma, lower acute care charges, decreased hospital length of stay, but also with a higher learning curve [75,76]. As such, the outcomes of these procedures are increasingly reported.…”

“…Among the techniques, the transforaminal is the most accessible at the beginning stages of learning the endoscopic procedures. 25 Initial training in the minimally invasive endoscopic techniques is practiced on cadavers and, later, in patients, always supervised and with all the safeguards necessary for the execution of the procedure.…”
