Learning From Errors in Radiology: A Comprehensive Review

Abstract: An important goal of error analysis is to create processes aimed at reducing or preventing the occurrence of errors and minimizing the degree of harm. The discovery of any errors presents an opportunity to study the types that occur and to examine their sources and develop measures to prevent them from recurring. The development of an effective system for detecting and appropriately managing errors is essential to substantially attenuate their consequences. At this stage, the error analysis process identifies … Show more

“…It is equally as important for radiology practices to internally adopt established peer review and audit procedures to detect trends and help identify opportunities to support radiologist education if unfavorable variability is encountered [7]. This recommendation aims to help reduce one of the most significant barriers of practice-based continued learning in radiology, which is inconsistent and often delayed feedback on reported findings.…”

“…We believe that it is through the just culture we would be able to provide our work environment to be less error prone and more error tolerant [23]. As mentioned by Pinto A et al, that identification and reduction of diagnostic error not only helps to reduce mortality and morbidity, but also affects the cost and length of hospital stay [24].…”

“…harm'' [1,2], are a daily risk in all fields of medicine, especially in emergencies where an accurate and quick diagnosis are required [3]. Fortunately, not all diagnostic errors result in serious injuries to patients because in most cases they are negligible or they are discovered and promptly corrected [2].…”

“…Fortunately, not all diagnostic errors result in serious injuries to patients because in most cases they are negligible or they are discovered and promptly corrected [2]. Many authors have tried to classify the different types of medical errors on the basis that it would help to decrease them and their negative consequences [1].…”
