Background: Transnational networks such as Communities of Practice (CoPs) are flourishing, yet their role in diffusing health systems reforms has been seldom investigated. Over the past decade, performance-based financing (PBF) has rapidly spread in Africa. This study explores how, through the PBF Community of Practice’s attributes, structure, and strategies, PBF diffusion was fostered in sub-Saharan Africa (SSA). Methods: Informed by the diffusion entrepreneurs’ (DEs) framework dimensions, we used a mixed methods convergent design to investigate how the attributes, structure, and strategies of this community fostered the diffusion of PBF. The quantitative strand of work included firstly a semantic discourse analysis of textual data extracted from CoP’s online discussion forum (n=1346 posts). Secondly, the relational data extracted from these 1346 forum posts was examined using social network analysis (SNA). We confronted these quantitative results with a thematic analysis of qualitative interviews (n=40) and data extracted from the CoP’s key documentation (n=17). Results: CoP members’ attributes included: representation systems anchored in clinical and economic sciences, strong expectations that the CoP would boost professional visibility and career, and significant health systems knowledge and social resources. The CoP’s core group, dominated by high-income country (HIC) members, critically matched PBF principles to major health systems issues in Africa. The broad consensus in online PBF thematic discussions created a strong sense of community, a breeding ground for emulation among CoP members. The CoP also sought to produce and promote experiential knowledge exchanges about PBF amongst African practitioners. Findings from network analyses showed that the promoted Africa-driven community was led by HIC members, although their prominence tended to decrease with time. Conclusion: This empirical research highlighted some of the constituting features, structure, and strategies of policy networks in influencing health policy diffusion. Despite good intentions to disrupt the established governance landscape, influential actors coming from HICs continued to drive the framing, and shaped health systems policy experimentation, emulation, and learning in African countries. Beyond mere knowledge exchange platforms, CoP can act as meaningful transnational policy networks pursuing the diffusion of health systems reforms, such as PBF.

En vue de sécuriser les terres rurales au Bénin, le gouvernement a instauré à travers la loi N° 2013-01 du 14 août 2013, une nouvelle méthode de formalisation des terres. Mais cette réforme semble rencontrer des réticences de la part des propriétaires fonciers. La présente étude fait part de la situation dans la moyenne vallée du Mono. 338 personnes composées des acquéreurs et vendeurs de parcelles ont été interviewées à travers la technique de boule de neige. Les informations sur les méthodes d’enregistrement des terres, les types de documents fonciers attestant leur droit de propriété et les raisons de leur réticence ont été collectées ; elles ont été ensuite traitées puis analysées et les récits ont été transcrits.Les résultats obtenus ont révélé que la formalisation légale des achats et ventes de parcelles auprès des autorités compétentes dans la moyenne vallée du Mono n’est pas toujours systématique et demeure encore timide dans certains villages. Pour cause, 43% des acquéreurs interviewés disposent d’une convention de vente contre 57% qui ont des reçus de vente en leur possession. De plus, la nouvelle méthode de formalisation instaurée par la nouvelle loi foncière semble aggraver la situation ; 65% des personnes enquêtées ont déclaré ne pas être informées de l’existence de cette méthode. Aussi, émettent-ils des réticences face à cette loi lorsqu’on la leur explique du fait des tracasseries, du coût élevé des formalités et de la lenteur administrative.

Rural land plans (RLP) and the systematic cluster approach (SCA) are the two main approaches used in Benin to secure rural land tenure. However, the contributions of these approaches to land tenure security in rural communities are mixed. This paper firstly identifies the main factors to be considered in conceptualizing rural land tenure security and secondly assesses the contributions of the two approaches in achieving rural land tenure security. The study is conducted in four communes of Borgou (a district in North Benin) that have benefited from both approaches to land tenure security. The dimensions of land tenure security were identified during focus group discussions and unstructured interviews with key informants. The contribution of the approaches is assessed using individual surveys from 742 beneficiaries of the approaches randomly selected, based on actors’ perceptions measured on the Likert scale. The nonparametric Friedman test was used to determine the average rank of each factor according to the RLP or SCA context. As a result, land tenure security must integrate nine factors, leading to two forms of land tenure security. Legal and institutional security includes land tenure norms, property rights, local land management institutions, and the land information system. Socioenvironmental security involves access and use rights, social peace, gender mainstreaming, and land conservation. According to stakeholders’ perceptions, RLPs mainly lead to legal-institutional security, whereas SCA combines and contributes to both forms of land tenure security. Rural smallholders will enjoy high levels of land security when these elements are in dynamic balance.
