Levels of Urgency for Orthopaedic Procedures: Reliability and Adoption of a Consensus-driven Classification

Abstract: Introduction: The current COVID-19 disease pandemic has delayed nonurgent orthopaedic procedures to adequately care for those affected by the severe acute respiratory syndrome coronavirus 2, resulting in a backlog in orthopaedic surgical care. As the capacity for orthopaedic surgeries expands or contracts, allocation of limited resources in a manner that adequately reflects medical necessity and urgency is paramount. An orthopaedic surgery-specific prioritization schema with proven reliability is l… Show more

“…Nevertheless, if classifications for dissimilar diseases are considered, resemblance with the Sander classification (0.58) and with toe deformities staging (0.61) may be noted. 7,27,43 Intraobserver reliability was found to be very good, with a Cohen kappa of 0.851 ( P < .001, 95% CI 0.777-0.926) sustaining the applicability of the classification in surgeons’ daily practice. When contemplating classes individually, different reliabilities could be demonstrated, portraying interesting scenarios.…”

“…A separate 4-tier system was developed recently for levels of urgency of orthopaedic procedures. 13 This system is time based and did not have specific elements but grouped them into phases, thus taking into consideration the phase, each tier would be allowed to proceed or not. These tiers were time-based, similar to the surgical acuity element proposed in this study.…”

“…The act of performing surgery is also known as a surgical procedure or operation [ 1 ]. Surgeries can be emergent or elective, while emergent surgeries are performed immediately, elective surgeries are postponed for a specific time and are performed when patient becomes due for procedure as ascertained by the examining surgeon during clinical examination of the patient [ 2 ].…”
