Limited sampling strategies for the estimation of atazanavir daily exposure in HIV‐infected patients

Abstract: Stepwise multiple regression analyses were applied to 44 atazanavir pharmacokinetic profiles from 44 HIV-1 infected patients concomitantly treated with raltegravir with the goal of identifying limited sampling strategies for the prediction of drug AUC(0-12) . Atazanavir trough-based equations failed to reliably predict daily drug exposure in patients with low drug bioavailability. Conversely, different algorithms based on few samples and associated with good correlation, acceptable bias and imprecision with th… Show more

“…8,9 Given the measured drop in concentrations of PIs in the third trimester, therapeutic drug monitoring (TDM) has been previously recommended to titrate drug doses according to measured levels. 10,11 However, TDM's effectiveness is questionable, as one-off measurements correlate poorly with a patient's 24 h drug exposure 12 and there is no evidence of TDM preventing viral breakthrough 13 or drug resistance. 14 Given the uncertainty of the use of TDM and its relationship with HIV in pregnancy, this analysis aims to describe which patients are selected for TDM and then review whether TDM effected HIV control and management during pregnancy as well as at delivery and in follow up maternally and neonatally.…”

“…Indeed “one-off” TDM measurements are thought to poorly correlate with overall drug exposure [1]. We aim to describe baseline demographic and clinical characteristics of pregnant women with HIV, and to compare their HIV control, management during pregnancy and neonatal outcomes with respect to whether TDM was performed.…”
