We report the case of a 43-year-old male with a littoral cell angioma. Clinical examination of the spleen revealed multiple cystic masses surrounded by normal splenic tissue. Histopathologic and immunohistochemical findings confirmed the diagnosis of a littoral cell angioma. The rarity in the literature of littoral cell angiomas is striking.Key words: Littoral cell angioma, Spleen, Vascular tumor ÖZET Bu yazıda splenektomi materyelinde littoral hücreli anjiom tanısı olan 43 yaşında bir erkek hasta sunulmuştur. Hastanın klinik incelemesinde dalakta çok sayıda kistik kitleden oluşan lezyon saptanmıştır. Histopatolojik ve immunohistokimyasal bulgular littoral hücreli anjiom tanısını desteklemiştir. Lezyon son derece nadir olması nedeni ile tartışmaya değer bulunmuştur.

This case report describes a novel presentation of littoral cell angioma (LCA) and lymphatic malformations involving the omentum and mesentery. To our knowledge, these 2 entities have not been reported in the same patient. A 1-month term infant male presented with chylous ascites. During his workup, imaging detected splenic nodules. Biopsies revealed that the nodules were LCA and the chylous ascites was secondary to microscopic mesenteric and omental lymphatic malformations. Evaluation for a secondary malignancy, an underlying immunologic defect, and genetic causes were unrevealing. The presence of LCA and lymphatic malformations in the same patient suggests a genetic link between these 2 rare vascular disorders and may help elucidate the etiopathogenesis of these 2 poorly understood anomalies.

Littoral cell angioma (LCA), a rare vascular neoplasm that occurs in the spleen, is difficult to definitively diagnose preoperatively because histological examination is the only accurate means of diagnosing this condition. Thus, the preoperative diagnosis is often incorrect. Splenectomy is the appropriate treatment and can be performed laparoscopically or by open surgery depending on the size of the tumor. Here we present a case of a 38-year-old man who presented with a history of slight non-specific abdominal distension for 2 months. An abdominal CT scan showed a huge spleen measuring 35×18 cm and containing multiple lesions, the largest being 11.3×9.2 cm. The patient underwent open splenectomy and recovered well. Histological examination showed LCA of the spleen. To our knowledge, this is the largest reported LCA thus far and may advance our understanding of this condition.
