Liver abscess caused by Clostridium bifermentans following blunt abdominal trauma

Abstract: A case of hepatic abscess and subsequent septicemia caused by Clostridium bifermentans is described. The abscess manifested itself on the third day after blunt trauma to the torso. The patient had nausea, vomiting, fever, evidence of hepatic dysfunction, and subphrenic gas. This case illustrates the association of hepatic abscess and blunt trauma to the torso.

“…Further, the results of biochemical characterization showed that isolate is negative for urease, inuline, trehalose and rhamnose fermentation and positive for indole, mannitol assimilation, citrate utilisation, glucose and mannose fermentation. C. bifermentans strains was reported as indole positive (Regan and Crawford 1994;Nachman, Kaul, Li, Slim, Filippo & Horn 1989). Similar results were documented for Clostridium which showed indole positive and urease negative (Brooks, Moss & Dowell 1969).…”

“…With regards to the control group C (sterile water rinse), the most unusual findings were the presence of Clostridum species, a rare oral isolate, in two samples from the control group. Clostridium bifermentans has been reported with serious but extremely rare complications such as metastatic osteomyelitis (Scanlan et al 1994) and metastatic liver abscess (Nachman et al 1989).…”

“…However, over the past three decades, several case reports have been published in the English language documenting a variety of clinical infectious due to this bacteria. C bifermentans has been described as a cause of septic arthritis, liver abscess following blunt abdominal trauma, abdominal abscess (in association with Cardiobacterium hominis ), delayed brain abscess due to a retained foreign body, metastatic osteomyelitis, soft tissue infection and endocarditis ( 7 , 9 15 ). Furthermore, in various published series of Clostridium species bacteremia, C bifermentans typically accounts for a small percentage of cases ( 16 18 ).…”

“…Older publications tended to regard C bifermentans as nonpathogenic ( 4 , 6 ). However, in more recent reports, this organism has been documented as a cause of septic arthritis, osteomyelitis, soft tissue infection, abdominal infections, brain abscess, bacteremia and endocarditis ( 7 , 9 18 ). The present article describes the third reported case of empyema caused by C bifermentans, and serves to further define the spectrum of illness due to this uncommon organism.…”
