Liver Disease From Asymptomatic Constrictive Pericarditis

Abstract: Congestive hepatopathy is a known complication of cardiac disease and is typically identified in the context of an established cardiac diagnosis and profound cardiopulmonary symptoms. We report the case of a 28-year-old man with liver disease secondary to asymptomatic constrictive pericarditis. This case highlights the need for gastroenterologists to consider occult cardiac disease as a cause of unexplained liver dysfunction.

“…High CVP pressure can be transmitted from the right heart atrium to the inferior vena cava and hepatic veins, which can result in liver congestion, cell hypoxia, and even fibrosis. Prothrombin time and TBIL can be high in this condition (12). Studies on the effect of CVP on liver function were mainly performed in liver transplant patients.…”

“…According to the mean CVP over 7 days, the enrolled cases were divided into three groups: low (<8 mmHg), normal (8)(9)(10)(11)(12), and high CVP (>12 mmHg). According to whether CVP dropped to less than 8 mmHg during 7 days, the enrolled cases were divided into two groups: CVP dropped to less than 8 mmHg and CVP did not drop to less than 8 mmHg.…”

“…Of 59 patients with advanced heart failure awaiting cardiac transplant or left ventricular assist device, 80% had hepatic fibrosis including 37% (n=22) with stage 3-4 fibrosis (4). Patients with CP may develop elevated serum liver enzyme levels, hepatic fibrosis, and subsequent liver failure due to chronic venous congestion if left untreated (11-15). …”
