Local therapy of rectal tumors

Abstract: In selected cases, local therapy of rectal carcinoma avoids high morbidity and mortality of the classical operation. Quality of life will be improved, especially if an artificial anus can be avoided. In case of recurrence, the chance of a secondary procedure for cure is not to be underestimated.

“…Subsequently, several authors presented their experience with TEM and carcinoids. 9,[117][118][119][120][121] Carcinoids eligible for local excision are typically smaller and less technically challenging than rectal adenocarcinomas. 118 Kinoshita and colleagues 117 reported their results in 27 patients with carcin oid tumours treated with TEM.…”

“…After this early success with benign disease, REVUE the technique has been subsequently used to remove early stage carcinomas and carcinoids of the rectum, for palliative resection of advanced rectal cancers and to surgically correct rectal prolapse and rectal diverticular disease. 9 The objective of this review is to describe the current status of TEM in the treatment of rectal lesions.…”

“…Virtually any adenoma up to 25 cm (proximal margin of the tumour) from the anal verge, including large circumferential lesions, can be removed by TEM [1]. Large sessile lesions may harbour an occult cancer in 30–50% of instances and hence, for these lesions a full thickness excision, traversing all layers of the bowel wall may be preferable [9].…”

“…Rectal lesions have always been difficult to approach and treat in view of the complex anatomy of the pelvis [1]. Traditionally, the treatment of rectal cancers and benign rectal tumours unsuitable for polypectomy is open surgery, either anterior resection or abdominoperineal resection.…”

“…The procedure allows a precise removal of tumors located 6-18 cm from the anal verge (6,7) and has been widely used for the evaluation and treatment of both adenomas and selected carcinomas, with minimal morbidity and mortality (8,9). The technique combines a meticulous dissection and suturing in the ano-rectum up to 18-20 cm from the anal verge under intense endoscopic visualization (10).…”
