Long-Term Comparison of Rib and Ear Cartilage Grafts in Autologous and Allogenic Fascia Lata: An Experimental Study in a White Rabbit Model

Abstract: This study shows the equivalency of diced cartilage in fascia grafts and isolated fascial grafts using allogenic fascia lata compared to autologous fascia. The type of cartilage used accounts for different long-term characteristics of diced cartilage in fascia grafts.

“…However, in comparison with other source of chondrocytes, CCs have stronger tendency of hypertrophy and calcification [ [7] , [8] , [9] , [10] ]. Hypertrophy is the terminal differentiation stage of growth plate chondrocyte, which is a necessary developmental stage in endochondral ossification during normal bone formation and growth [ 11 ].…”

“…34 Wrapping diced cartilage in an Allo-Derm sleeve or in Tutoplast-processed fascia lata instead of temporalis fascia has been said to be a viable alternative. 21,22 In rats, both solid and diced cartilages wrapped in amniotic membrane had a higher success rate than bare cartilage grafts. 40 To overcome adding a second surgical site to the procedure by harvesting temporalis fascia, diced cartilage has been successfully wrapped in rectus abdominis fascia and pectoralis major fascia being in the costal cartilage donor area.…”

“…18 Homologous lyophilized bone and fascia lata have also been used with recently renewed interest in the use of processed fascia lata. [19][20][21][22] To overcome the incongruent shape of solid cartilage grafts, septal cartilage in particular, weakening of grafts by crushing has been widely used in the past. The degree of crushing is known to have a strong impact on resorption rates with intact or mildly crushed cartilage having a better survival in retrospective clinical and animal studies.…”

“…15,16 To avoid a second operative site and related potential complications, diced cartilage has also been successfully used with processed fascia lata for augmentation rhinoplasty. 17,18 However, Lan 19 and Yang 17 observed resorption and replacement of inserted fascia lata by the host's own fibrous tissue leading to scar contraction of the overlying skin and soft-tissue envelope. The senior author observed a significant shortening of the nose in 1.2% of patients who have been treated with processed fascia lata (personal communication with Yang YJ).…”
