Long-term outcome of mitomycin C-augmented needle revision of trabeculectomy blebs for late trabeculectomy failure

Abstract: Bleb needling augmented with MMC can achieve a significant reduction in IOP in eyes with late trabeculectomy failure. By 3 years however, 59% of eyes were classified as failures, with half requiring further glaucoma surgery or laser. Patients undergoing this procedure should therefore be warned of the likelihood of requiring further intervention, and a repeat antimetabolite-augmented trabeculectomy or glaucoma tube shunt surgery in the first instance can be considered instead.

“…As antimetabolites were applied to keep the wound from fibrosis, a similar effect on IOP reduction further validated the efficacy of bleb needling itself. Although long-term studies had proved that the impact could sustain for more than three years but gradually decrease in success rate with time [ 13 , 15 , 24 , 29 ], an overall analysis of the IOP reduction level had shown a stable trend in more than six months in our results. Therefore, we considered that the present meta-analysis provides generalizable information for bleb needling in clinical settings.…”

“…IOP reduction levels at six months, one year, two years, and last visit after bleb needling were analyzed. Data at one year were regarded as last-visit data for the studies with loss of >50% cases at the last visit [ 12 , 15 17 ]. Combined analysis with a random-effect model showed a significant reduction in IOP at a different timeline ( p < 0.001).…”

“…Studies evaluating success of bleb needling for trabeculectomy, ExPress shunt, and XEN gel stent show up to 40% success [ 7 9 ]. However, a number of studies evaluating the clinical outcomes after placement of a XEN gel stent report higher early postoperative bleb needling rates up to 44.1% at 1 year [ 4 , 10 12 ].…”
