Long-term outcomes of endoscopic submucosal dissection for early gastric cancer: a multicenter collaborative study

Abstract: ESD for expanded-indication lesions of EGC is considered an effective therapy associated with an extremely low rate of metastatic recurrence on long-term follow-up. However, fully informed consent concerning the risk of metastatic recurrence should be obtained before ESD, and close postoperative follow-up is essential.

“…The J‐WEB/EGC study is the first multicenter prospective study conducted in the context of ‘real‐world’ settings to demonstrate a favorable short‐term outcome for gastric ER (mostly ESD) among a large number of EGC lesions based on all categories of the absolute and expanded indications . There are a number of published reports on the short‐term and long‐term outcomes of ESD for EGC, but most of the reports are retrospective studies from single centers . There were two multicenter prospective studies about the clinical outcomes of ESD for EGC; however, these multicenter prospective studies did not include all EGC lesions meeting the absolute and expanded indications .…”

“…Recently, a number of reports have been published regarding the short‐term and long‐term outcomes of EMR and ESD for EGC, but most of these reports were retrospective studies from single centers . A Japanese multicenter prospective cohort study of ER for EGC using a Web registry system (J‐WEB/EGC) has been conducted to determine the short‐term and long‐term outcomes based on the absolute and expanded indications .…”

“…Indications for endoscopic submucosal dissection (ESD) are based on the criteria of the Japanese Gastric Cancer Treatment Guidelines 2014 (ver. 4), and extended indications for ESD in patients with early gastric tumors without lymph node metastasis have been clinically discussed 3, 4. According to these guidelines, standard gastrectomy with lymphadenectomy is still recommended for patients with tumors and no indications for ESD 3.…”

“…For instance, in Japan, endoscopy has been recently accepted as a primary tool in population-based gastric cancer screening 1 . Endoscopy is also a vehicle for modern gastric cancer treatments, such as endoscopic submucosal dissection (ESD) 2 3 4 . Although advanced gastric cancer still has a poor prognosis, patients with early-stage disease treated with ESD are likely to have favorable outcomes 3 4 5 .…”

Although advanced gastric cancer still has a poor prognosis, patients with early-stage disease treated with ESD are likely to have favorable outcomes 3 4 5 . For that reason, early detection of gastric cancer is important and endoscopy is a critical tool for its early detection, diagnosis, and treatment.…”
