Long-term results of photodynamic therapy for the treatment of choroidal neovascularization secondary to angioid streaks

Abstract: PDT with verteporfin does not seem to be a good treatment for stabilization of VA and lesion size in CNV secondary to AS, although the overall results seem to be slightly better in the group of patients treated with PDT than in the control group.

“…Nevertheless, in the group of juxtafoveal CNV (7 eyes) 4 eyes progressed to a subfoveal location during the follow-up of 12 months. Again a reduction of mean BCVA from 20/126 at baseline to 20/500 after a mean follow-up of 18 months was reported by Arias et al [77]. A long-term follow-up from 12 to 50 months (mean 26.1 months) of 15 eyes (10 with subfoveal, 5 with extra- or juxtafoveolar CNV) was presented by Heimann et al [35].…”

“…Other studies with longer follow-up also report enlarged CNV or disciform lesions in most patients with poor visual acuity of 20/600. 42,43 A study showed that the mean visual acuity decreased from 20/ 32 to o20/200 in nearly half the patients and only 13% was noted to have X20/63 at final follow-up. 44 Anti-angiogenic agents Owing to the rarity of this condition, single case reports are the prevalent source of treatment outcome data.…”

“…Typically, the other causes of CNV affect working-age persons, are multifocal, sometimes juxta or extra-foveal, and can be recurrent [1,3]. In the past, lesions of CNV related to angioid streaks had a particularly poor prognosis with bad responses to photocoagulation [12] or photodynamic therapy (PDT) [13,14,15,16]. In the literature there are only a few reports of series of patients treated off-label with bevacizumab [17,18,19,20,21,22,23] or ranibizumab [23,24,25].…”
