Introduction: Although the liver and lung are the most frequently affected organs in cystic echinococcosis, the cysts may develop in any viscera and tissues. Breast is a rare primary localization with few cases described in the literature. We present an updated and systematic review and discuss the possible mechanisms of spreading, diagnostic and treatment options.Materials and methods: We performed a literature search in PUBMED using the key words ‘hydatid disease’, ‘cystic echinococcosis’ and ‘breast echinococcosis’ without time limitation. Only studies reporting breast cystic echinococcosis were included.Results: Overall, 121 cases with cystic echinococcosis and 2 with alveolar echinococcosis were reported. A total of 52 cases were included in the analysis. The mean size of cysts was 5.5 cm (range 1.7-12). The most common clinical presentation was painless lump presented from 4 months to 19 years before the final diagnosis. Most cases had isolated breast CE, few cases had synchronous localizations – femoral, thigh and lung, and previous liver CE. Most were active CL and CE1-2 cysts (72%). Ultrasound was used in 83%, followed by mammography (35%). Fine needle aspiration was reported in 27 cases with positive finding in 59%.Conclusions: In cases with cystic breast lesions from endemic regions we recommend the US as a gold standard. CT and MRT are more accurate but expensive tools without the potential to change the surgical tactic. In contrast to the other localizations of CE, complete excision of the cysts is the best diagnostic and treatment approach.

La localisation mammaire du kyste hydatique est exceptionnelle. De ce fait, le diagnostic est difficile avant l'examen anatomopathologique. Nous rapportons une observation d'un cas de kyste hydatique du sein chez une femme de 32 ans, qui consultait pour un nodule du sein, dont l'imagerie montrait une lésion en rétro-aréolaire du sein gauche, homogène, ovalaire et de contours réguliers. L'examen anatomopathologique objectivait un kyste hydatique à localisation mammaire. Les suites opératoires étaient simples. L'hydatidose est une maladie ubiquitaire, pouvant atteindre tous les organes. Le diagnostic peut être évoqué devant une masse kystique du sein avec des aspects très évocateurs à l'imagerie. La confirmation du diagnostic n'est confirmée qu'après une cytoponction ou une chirurgie d'exérèse qui réalise le traitement de cette pathologique. La négativité du bilan d'extension hydatique permet de retenir une localisation primitive de l’échinococcose.

Introduction Due to its rarity, there is limited information regarding breast hydatid disease, mostly derived from case reports. This study aims to review the clinical presentation and management of the disease systematically. Methods The PubMed/Medline and Google Scholar were searched with the following keywords: (Breast OR Breasts OR "mammary gland" AND hydatid OR hydatidosis OR echinococcus OR echinococcal OR echinococcosis OR "E. granulosus"). Only English-language studies published in legitimate journals were included. Additional inclusion criteria involved a confirmed diagnosis of breast hydatid cyst, details on presentation, cyst characteristics, and management. Results Most cases were reported in India (32.6%) and Turkey (25.6%). The mean age of the patients was 40.88 ± 15.7 years, and all 43 patients were female. Most (69.8%) were asymptomatic, while 25.6% reported pain and 2.3% reported dry skin. Surgery with albendazole was the treatment of choice in 51.2% of the cases, followed by surgery alone in 39.5%. Most cases (65.1%) recovered without complications, whereas one did not and was under follow-up. The recovery status was not mentioned in 14 cases (32.6%). Conclusion Breast hydatid cyst is an extremely rare finding that can be asymptomatic in most instances. Surgery combined with albendazole may be the most successful treatment approach.
