Chlamydia trachomatis Infections and Subfertility: Opportunities to Translate Host Pathogen Genomic Data into Public Health

Abstract: Chlamydia trachomatis (CT) infections in women can result in tubal pathology (TP). Worldwide 10-15% of all couples are subfertile, meaning they did not get pregnant after 1 year. Part of the routine subfertility diagnostics is the Chlamydia Antibody Test (CAT) to decide for laparoscopy or not in order to diagnose TP. The CAT positive and negative predictive value is such that many unneeded laparoscopies are done and many TP cases are missed. Addition of host genetic markers related to infection susceptibility … Show more

“…In the literature, higher PPVs and NPVs, of respectively 95% and 70%, for TFI have been described. However, these values relate to MIF serology instead of ELISA serology (Keltz, Gera and Moustakis 2006;Lal et al 2013). Land et al (2003) described a PPV and NPV of the Medac pELISA for TFI of respectively 38% and 90% in subfertile women who underwent laparoscopy.…”

“…These studies are described below; however, for the sake of brevity, we have documented the sample sizes, statistics, and details of the alleles in Table 1. Note that since a comprehensive review of this topic (mentioned above) has recently been published, this section is brief (108).…”

“…Indeed, regarding genetic susceptibility to chlamydial infection, two good reviews are available, by Morrè et al and Lal et al (222,223). In cases of persistent infection, what are the characteristics of immunity that allow the infection to persist?…”
