Tribolium confusum (Confused Flour Beetle, Rice Flour Beetle) – An Occupational Allergen in Bakers: Demonstration of IgE Antibodies

Abstract: Specific IgE to proteins from Tribolium confusum (TC), a flour beetle, was detected in 9/125 sera of subjects exposed to rye and wheat flour. TC RAST was not inhibited by Dermatophagoides pteronyssinus, rye or wheat flour. Immunoblot experiments showed specific binding to three proteins from adult TC or pupae, not present in rye or wheat flour. These findings suggest that TC might act as an occupational allergen in a proportion of bakers.

“…Likewise, among 133 bakers tested by skin prick, 8.3% were positive to T. confusum (228). Schultze-Werninghaus and colleagues later reported that IgE in the sera of subjects exposed to rye and wheat flour bound to three T. confusum-specific proteins (229).…”

“…There are a few reports of IgE-mediated respiratory allergy to confused flour beetle [51][52][53][54]. Among the 1,040 patients with perennial asthma and/or rhinitis and with proven sensitization to indoor allergens, 16.5% had positive skin prick tests and 30.7% positive intradermal tests to whole-body extracts of T. confusum [51].…”

Positive skin prick test was also noted in 8.3% of 133 bakers [52], and specific IgE to T. confusum proteins was detected in 9 of 125 sera of subjects exposed to rye and wheat flour [53]. A case of IgE-mediated respiratory and skin allergy as a result of occupational exposure to T. confusum has been reported recently [54].…”

“…Different allergens including storage mites (4), flour beetles and moths cause occupational asthma in bakers (5) and workers exposed to grains (6,7). This study reports the presence of Bla g 2 allergen, derived from German cockroach, in flour samples.…”
