Lung Cancer in Patients with Idiopathic Pulmonary Fibrosis.

Abstract: We investigated lung cancer in 99 patients with idiopathic pulmonary fibrosis (IPF). Lung cancer was found in 31 (31.3%) of 99 patients with IPF. Most (87.9%) tumors, including squamous cell carcinoma, were observed in the peripheral region of the lung, whereas the distribution of histologic types of cancers was similar to that seen in ordinary lung cancer. Peripheral tumors were frequently seen in the lower lobe, where fibrotic shadow was prominent. However, the severity of fibrosis was not related with the p… Show more

“…In previous literature, IPF-CA was reported to be more frequent in the periphery of the lung and the lower lobe, where the fibrosis was predominant, suggesting a direct relationship [5,6,19]. The peripheral location of the lung cancer was reported as 88% by NAGAI and CHIYOTANI [5], 91% by MIZUSHIMA and KOBAYASHI [2] and 66% by LEE et al [19].…”

“…The peripheral location of the lung cancer was reported as 88% by NAGAI and CHIYOTANI [5], 91% by MIZUSHIMA and KOBAYASHI [2] and 66% by LEE et al [19]. If fibrosis is directly related to the development of the cancer, cancer will develop inside or in close proximity to the area of major fibrosis.…”

“…The prevalence of lung cancer among IPF patients was reported in 15 studies [26,28,34,48,[82][83][84][85][86][87][88][89][90][91][92] (online supplementary table S4 and figure 5) and was typically lower than the prevalence of other respiratory comorbidities of interest. Analyses of insurance claims and electronic medical records databases from the USA and the UK estimated the prevalence of lung cancer to be 3-4% [26,82,83].…”

“…In fact, it is mainly in systemic sclerosis (62) and cryptogenic fibrosing alveolitis (63,64) that any larger number of lung cancers are found. The risk of lung cancer in silicosis is also moderately increased (relative ratio 3.4 for smokers with silicosis and 1.7 for nonsmokers) (65)(66)(67).…”

“…The typical cancers in systemic sclerosis are peripheral adenocarcinomas or bronchioalveolar cell carcinomas (62), but those in idiopathic fibrosis are similar to the bronchial carcinomas found in smokers (63,64,68). Of special interest is the review by Mizushima & Kobayashi (68).…”
