Lymphogranuloma venereum: an emerging problem in Poland

Abstract: Lymphogranuloma venereum (LGV) is a sexually transmitted disease that increases in incidence, particularly in more developed countries worldwide. LGV is caused by Chlamydia trachomatis serovars/genovars L1-3, including their subvariants, and in Europe mostly affects men who have sex with men (MSM). It can be asymptomatic but has now emerged as a frequent cause of severe proctitis/proctocolitis, especially in MSM. LGV has often been misdiagnosed as C. trachomatis serovars/genovars D-K infection. It is essential… Show more

“…In 2018, two additional cases of C. trachomatis L2b were confirmed in MSM men aged 47 and 36, HIV negative and without concomitant sexually transmitted diseases. Both patients had enlarged inguinal lymph nodes and lower gastrointestinal complaints such as anal and rectal mucosa ulcerations and bleeding [15]. All patients were treated with doxycycline in accordance with the current recommendations, with a satisfactory therapeutic effect.…”

“…W 2018 roku potwierdzono dwa dodatkowe przypadki C. trachomatis L2b u mężczyzn MSM w wieku 47 lat i 36 lat, HIVujemnych oraz bez współistniejących chorób przenoszonych drogą płciową. U obu pacjentów stwierdzono powiększone węzły chłonne pachwinowe oraz dolegliwości z dolnego odcinka przewodu pokarmowego w postaci owrzodzeń śluzówki odbytu i odbytnicy oraz krwawień [15]. U wszystkich pacjentów włączono leczenie doksycykliną zgodnie z obowiązującymi rekomendacjami z zadowalającym efektem terapeutycznym.…”

“…In 2003, endemic cases of LGV proctitis and proctocolitis were reported in Europe, clearly associated to HIV-positive men who have sex with men (MSM) ( 10 , 11 ). Since then, similar LGV cases have been increasingly reported mainly in the MSM community of metropolitan areas in Europe, the USA, Canada and Australia ( 12 16 ). Interestingly, the ongoing epidemics of LGV in developed countries displays particular epidemiologic features, including a strong association to MSM and HIV co-infection, proctitis sometimes mimicking Crohn's disease symptoms as the main clinical finding, a significant proportion of asymptomatic cases likely contributing to LGV transmission and, surprisingly, a markedly increased proportion of anorectal compared to genital infections ( 17 21 ).…”
