Magnetic Resonance Imaging of the Idiopathic Inflammatory Myopathies

Abstract: Idiopathic inflammatory myopathies are chronic diseases clinically characterized by symmetrical proximal muscle weakness. MRI has assumed a major role in the evaluation and management of these conditions. It is sensitive to the presence of inflammation and edema, especially with incorporation of fat suppression sequences, so it is a useful tool for establishing an early diagnosis, for evaluating the extent and number of lesions, and for determining the right site for biopsy. The noninvasive nature of the proce… Show more

“…Routine MRI performed with T1-weighted and STIR sequences is more sensitive but less specific than biopsy in diagnosis; it is advantageous for optimizing efficacy of classical diagnostic procedures [14]. Its importance is progressively growing, since it allows to noninvasively characterize the distribution and pattern of parenchymal changes and to monitor the disease progression, which has important implications for treatment [13, 15]. …”

“…WB MRI (Fig. 8) demonstrates diffuse, symmetric muscle edema centered predominantly in the shoulder and hip girdles, but also involving the neck flexors, erector spinae, and psoas muscle groups with no significant subcutaneous edema (O'Connell et al 2002;Curiel et al 2009;Adams et al 1995).…”

“…The etiology of dermatomyositis is thought to be a humorally mediated vascular injury, with perivascular inflammation and perifascicular atrophy resulting in microangiopathy and myocyte necrosis (Dalakas and Hohlfeld 2003;Bohan and Peter 1975;Curiel et al 2009). Dermatomyositis has a bimodal pattern of distribution.…”
