Magnetic resonance imaging of uveitis

Abstract: Increased uveal tract enhancement is a common finding in patients with uveitis, regardless of anatomic distribution and etiology. MRI can also further evaluate complications of uveitis and help differentiate it from masquerade syndromes.

“…Uveitis is an inflammation or infection of any part of the uveal tract, comprising the iris anteriorly, the ring-shaped ciliary body, and the choroid posteriorly. When inflammatory, uveitis is either idiopathic or in the context of a systemic disease, such as HLA-B27-associated seronegative spondylarthropathies, rheumatoid arthritis, and sarcoidosis [15]. The diagnosis of uveitis is predominantly based on clinical symptoms, such as pain, photophobia, decreased vision, and, on slit-lamp biomicroscopy and fundoscopy, the presence of cells and flare in the vitreous and anterior chamber respectively.…”

“…MRI signs of uveitis include increased enhancement and/or thickening of the uveal tract, subretinal effusions, and vitreous humor signal abnormalities, with no DWI restriction (Fig. 2) [15]. On imaging, inflammatory scleritis and infectious scleritis are alike, as are inflammatory and infectious uveitis.…”

“…Vogt-Koyanagi-Harada (VKH) disease, an inflammatory condition resulting from autoimmune targeting of melanocytes and characterized by bilateral granulomatous panuveitis, has been reported to cause subretinal effusions with associated restricted diffusion. 2 Though the restricted diffusion appears to be caused by bilateral symmetric increased uveal “cellularity,” the disease should be entertained in the differential diagnosis in the adequate clinical scenario.
