Major depression in coronary artery disease patients with vs. without a prior history of depression.

Abstract: Major depression (MD) is common in patients with coronary artery disease (CAD). Some of these patients have a history of prior depressive episodes, whereas others experience their first episode around the same time that their CAD is diagnosed. The purpose of this study was to determine whether there are systematic differences between these two subgroups of depressed patients. Of 39 patients with recently diagnosed CAD who met DSM-III-R criteria for MD, 17 (44%) had a prior history of MD. This subgroup had a hi… Show more

“…These results arise from direct visualization of the coronary vessels and support findings from one previous study which has shown more severe CAD in the incident depression subtype although only at trend level (Freedland, 1992). Even though the sample size in the present study was modest, the prevalence of the two depression types is comparable to that in other studies (Grace et al, 2005;de Jonge et al, 2006).…”

“…In a sample of 39 recently diagnosed CAD patients who met criteria for MDD, those with a depression history had less severe CAD in comparison to those without a depression history (Freedland et al, 1992). CAD severity was assessed by direct visualization of coronary arteries via angiogram, recorded as the number of occluded coronary arteries (>50% stenosed).…”

“…Freedland et al [38 ]reported prevalences of 44% for recurrent major depressive disorder in newly diagnosed patients with CHD. A recently published study by Bankier et al [39 ]examined the prevalence of psychiatric disorders assessed with a diagnostic psychiatric interview in stable CHD outpatients and reported a substantial proportion (15%) of patients with dysthymia.…”

“…Depression is not only seen in patients after an acute cardiac event, but in between 17 and 23% of those with newly diagnosed or chronic disease (cf., Carney et al, 1987). In addition, it appears that for as many as 50%, the depression predates the initial diagnosis of CHD (Freedland, Carney, Lustman, Rich, & Jaffe, 1992). Fewer studies have been conducted with patients undergoing coronary bypass surgery (CABG); however, research has found clinically meaningful depression in close to 50% of patients awaiting CABG, and in over 50% of patients up to six months after CABG (White et al, 1995).…”
