Making every drop count: reducing wastage of a novel blood component for transfusion of trauma patients

Abstract: Recent research demonstrates that transfusing whole blood (WB=red blood cells (RBC)+plasma+platelets) rather than just RBC (which is current National Health Service (NHS) practice) may improve outcomes for major trauma patients. As part of a programme to investigate provision of WB, NHS Blood and Transplant undertook a 2-year feasibility study to supply the Royal London Hospital (RLH) with (group O negative, ‘O neg’) leucodepleted red cell and plasma (LD-RCP) for transfusion of trauma patients with major haemo… Show more

“…If the results of upcoming whole blood trials show no benefits with a whole blood component in prehospital environments, then the RCP component could become the frontrunner for prehospital services in the UK to treat traumatically injured bleeding patients, considering its logistical benefits and equivalent effect on outcomes with current standard practice. In this scenario, extending its shelf life to beyond 14 days would be a priority for reducing hospital blood wastage [ 18 , 29 ].…”

“…Finally, extending the use of blood for major traumatic haemorrhage into the pre‐hospital environment may seem clinically intuitive, 131 but has significant implications for transfusion laboratory services at a local and national level 131 . Such challenges include the demand for use of 'universal' blood group components, the potential for wastage, and the need for regulatory compliance and associated workload 132 . Further information on the selection of blood for pre‐hospital transfusion and transferring of blood between hospitals is provided in the Supplement.…”
