Assessment of microbial bioburden on portable medical equipment in a hospital setting

Objectives: Although routine disinfection of portable medical equipment is required in most hospitals, frontline staff may not be able to disinfect portable medical equipment at a rate that adequately maintains low bioburden on high-use equipment. This study quantified bioburden over an extended time period for two types of portable medical equipment, workstations on wheels and vitals machines, across three hospital wards. Methods: Bioburden was quantified via press plate samples taken from high touch surfaces on 10 workstations on wheels and 5 vitals machines on each of 3 medical surgical units. The samples were taken at three timepoints each day over a 4-week period, with random rotation of timepoints and portable medical equipment, such that frontline staff were not aware at which timepoint their portable medical equipment would be sampled. The mean bioburden from the different locations and portable medical equipment was estimated and compared with Bayesian multilevel negative binomial regression models. Results: Model estimated mean colony counts (95% credible interval) were 14.4 (7.7–26.7) for vitals machines and 29.2 (16.1–51.1) for workstations on wheels. For the workstations on wheel, colony counts were lower on the mouse, 0.22 (0.16–0.29), tray, 0.29 (0.22, 0.38), and keyboard, 0.43 (0.32–0.55), when compared to the arm, as assessed by incident rate ratios. Conclusions: Although routine disinfection is required, bioburden is still present across portable medical equipment on a variety of surfaces. The difference in bioburden levels among surfaces likely reflects differences in touch patterns for the different portable medical equipment and surfaces on the portable medical equipment. Although the association of portable medical equipment bioburden to healthcare-associated infection transmission was not assessed, this study provides evidence for the potential of portable medical equipment as a vector for healthcare-associated infection transmission despite hospital disinfection requirements.

Introdução: A desinfecção é um processo físico ou químico aplicado em superfícies fixas, como materiais e equipamentos, que aniquila microrganismos patogênicos. Objetivo: Descrever a importância da desinfecção da superfície do manguito e do aparelho oscilométrico automático de medida indireta da pressão arterial. Métodos: Revisão integrativa realizada durante o mês de agosto de 2020 que incluiu artigos publicados na íntegra, no período de 2015 a 2020, publicados em português, inglês e espanhol, nas bases Lilacs, PubMed, Cinahl, Web of Science, Scopus, BDEnf e EMBASE, a qual buscou responder a seguinte pergunta “qual a importância da desinfecção da superfície do manguito e do aparelho oscilométrico automático de medida indireta da pressão arterial?”. Resultados: Sete artigos selecionados, publicados em cinco países diferentes que ressaltam a importância de desinfecção de superfícies, principalmente de uso comum. Salienta-se a necessidade de feedbacks aos profissionais de saúde quanto às condições de desinfecção e transmissões de infecções. Conclusão: As evidências identificaram falhas no processo de desinfecção de superfícies tanto dos manguitos quanto do aparelho, incluindo os esfigmomanômetros, e riscos de transmissão de patógenos infecciosos, destacando a desinfecção com detergentes e desinfetantes de fácil acesso.

Enteral tube feeding is the delivery of nutritionally complete feed via a tube into the gut. It is used for patients who are unable to meet their nutritional needs orally. Enteral feeding can be given through a variety of different tubes that access the gastrointestinal tract either via the stomach or the small bowel. The contamination of enteral feed can often be overlooked as a source of bacterial infection. Enteral feeds can become contaminated in a variety of different ways. Most often infections result in extended lengths of stay in hospital and patients also need additional therapies and treatments in order to resolve these infections. Healthcare-associated infections not only affect the patients who acquire them but also have an impact on the staff involved in their care. Each acute trust will have its own local policies and guidelines regarding enteral feeding and infection control and prevention. These local documents will be based on national initiatives and guidelines. It is important for nurses to refer to their local policies and guidelines before they start a patient on enteral feeding to ensure that they are doing so in the safest manner possible. Nurses' practice is key to preventing bacterial contamination in such patients.
