Malignant mesothelioma secondary to chronic inflammation and old scars. Two new cases and review of the literature

Abstract: Old scars of the pleura, for example from chronic empyema or therapeutic pneumothorax, can cause a malignant mesothelioma appearing some decades later. The authors describe two cases of primary pleural tumors, which probably had such an etiology. A literature review revealed 20 further cases, the majority of which were squamous cell carcinomas. Whether these tumors developed from metaplastic mesothelium or inplanted cells from the skin is not clear.

“…There were also no individuals with MM and a nonmesothelial neoplasm who were known to have conditions that cause chronic serosal inflammation, which has been suggested as a possible inducer of MM. [21][22][23][24] Interestingly, of the types of tumors overrepresented among individuals with MM and second neoplasms in this study, a number are either components of the BAP1 tumor predisposition syndrome or have been reported in BAP1 mutation carriers. 1,3 Recognized BAP1 tumor predisposition syndrome-related nonmesothelial tumors that were overrepresented among individuals with MM in our study population include clear cell renal cell carcinoma, lung carcinoma, ocular melanoma, and meningioma.…”

“…In the pleura, MM has occurred in individuals without an elevated lung tissue asbestos burden and/or evidence of asbestosis decades after therapeutic pneumothorax or thoracic plombage with Lucite spheres for pulmonary tuberculosis 4 6. Pleural MM has also been reported in individuals with long-standing chronic empyema or recurrent pleural effusions who had no documented exposure to asbestos 3 5.…”

“…The low frequency of pleural NHL in Western countries might be explained by the rarity of chronic pyothorax in these cases. Differential diagnoses such as malignant mesothelioma [6], soft tissue sarcoma [7], and squamous cell carcinoma [8] have been reported to complicate chronic pyothorax. However, lymphomas are the most frequent tumour arising in such a context [2].…”
