Malignant presternal goitre

Abstract: A B S T R A C TGoitres usually enlarge and descend caudad into the substernal space and are not palpable. We report on a patient whose goitre spread downward but anterior to the sternum. The thyroid mass was subsequently removed and was proven to be a papillary thyroid carcinoma. The mechanism by which a presternal goitre develops is probably due to invasion and erosion of the strap muscles and the cervical linea alba. The clinical implication of this presentation is complete extirpation of the presternal goit… Show more

“…Invasion to the subcutaneous tissue, larynx, trachea, oesophagus, and recurrent laryngeal nerve was considered as T4 in the Tumor Node Metastasis (TNM) system 1 2. Different from previous reports showing extension to the ventral of thoracic wall,3 4 our case had more rapid period of disease progression. Pathophysiology of sternal extension of thyroid cancer is largely unknown due to limited reported cases.…”

“…Colonic leiomyomas are very rare, accounting for only 3% GI smooth muscle tumors, and usually located in the descending or sigmoid colon [3][4][5][6]. However, polypoid leiomyomas in the colon and rectum are increasingly detected during colonoscopy [7].…”

“…Substernal goiter is more common due to the negative intrathoracic pressure during inspiration, which can drag down the thyroid along with gravity effect. 4 Presternal goiter, on the other hand, occurs probably due to migration and growth of thyroid cells into the cervical linea alba between the strap muscles, passing by the pretracheal fascia that encloses the thyroid retrosternally. 5 US (ultrasound) is one of the best exams to detect and to characterise the thyroid.…”

“…Due to this, goiters usually migrate down into the superior mediastinum -known as substernal goiters. [3][4][5] But in rare case, the thyroid enlargement migrates in an anterior way, in front of the stern, causing the presternal goiter. The pathogenesis behind this type of alternative migration is not known yet.…”
