Management of a Laryngeal Venous Malformation With Nd:YAG Laser and Bleomycin Sclerotherapy

Abstract: Literature describing neodymium:yttrium‐aluminum‐garnet (Nd:YAG) photocoagulation and sclerotherapy for laryngeal venous malformations (VMs) is sparse. Here we present a case in which an extensive laryngeal VM in a 28‐year‐old female was managed through a combination of four serial Nd:YAG laser photocoagulation sessions and four bleomycin injections over the course of 2 years. The treatment plan resulted in resulted in noticeable lesion ablation, mucosalization, and significant improvement in symptoms. To our … Show more

“…Our case adds to the evidence for even large LVMs in adult populations being treated with sclerotherapy. Final removal of the lesion has been previously performed with neodymium-doped yttrium aluminium garnet laser;1 however, our case demonstrates that ultrasonic excision is also safe and effective.…”

“…Laryngeal vascular malformations (LVMs) are rare, and the literature is limited to small case series and reports. They are however usually symptomatic, and patients can present with exertional fatigue, hoarseness or bleeding 1 2. The LVMs reported in the literature vary in terms of size and aetiology.…”

“…Sclerotherapy is the most commonly reported method for treating LVL. Sclerotherapy causes an obliteration of dysmorphic vessels endothelial cells, resulting in the devascularization of the vascular lesions 13 . Its main advantages are simple procedure and low risk of trauma.…”
