Abstract: In this study, we present a case of Grade IV diabetic foot ulcer (DFU) and its management with Vrana ropana chikitsa along with Ayurvedic precision medicines. A 72-year-old male patient having grade IV DFU was given the Vrana ropana chikitsa with precision medicines. A lactational breast abscess is the most common complication met in breastfeeding women, which serves as the common cause of discontinuation of breastfeeding. The conventional treatment choice for breast abscess is incisive drainage that is reportedly associated with few limitations. Ayurveda offers a multidimensional approach in the treatment of Vidradhi, which resembles abscess on clinical correlation. A case of a 25 year old multiparous woman, eight months postpartum with Stana vidradhi (~lactational breast abscess) in Pachyamanavastha (~transforming stage of disease) presented to the outpatient department. She was treated with Apatarpana (~depleting therapy), internal Ayurveda medicines, and in-situ non-invasive procedures for seven days. This intervention protocol resulted in a marked resolution of complaints along with a completely healed wound without a scar on the follow-up visit after one week. The present case offers a promising alternative for lactational breast abscess with the use of multidimensional noninvasive therapy resulting in shorter healing time and satisfactory cosmetic outcome.

Lichenoid dermatitis is a type of chronic eczema. Eczema can be correlated with Vicharchika (~wet eczema) described in Ayurveda. In acute conditions, this disease responds well to treatment, but in chronic presentations, it is difficult to manage due to its recurrent nature. Panchakarma (~five biopurification therapies) is well-known measure to improve the quality of life and prevention of recurrence in chronic diseases. In this case report, a 27-year-old male patient presented with chronic lichenoid dermatitis having symptoms of Kandu (~itching), Daha (~burning sensation), Ruja (~pain), Pidaka (~papules/eruptions), Srava (~discharge), Vaivarnya (~discoloration), and Rukshata (~dryness) treated with Ayurvedic interventions. Virechana (~therapeutic purgation) along with Ayurvedic oral medicines and wholesome dietary regimen were given for 12 weeks. Encouraging relief in various symptoms was observed after treatment. Rasayana (~rejuvenation) was also given to patient for prevention of recurrence. The patient has shown well tolerance and satisfactory adherence to treatment. No adverse event was reported during the course of therapy. After three months of treatment, disease progression was fully stopped and lesions resolved completely.

Management of Medoja Granthi (pedunculated lipoma) by Apamarga Ksharasutra ligation and Ayurveda medications: A case report

INTRODUCTION: A pedunculated lipoma is a benign fat tissue tumor conventionally treated by surgical excision. This condition can be correlated with Medoja Granthi. Ksharasutra ligation is an effective minimal invasive parasurgical procedure suggested for its management in Ayurveda. MATERIALS AND METHODS: In this case report, a 65-year-old female patient came with complaints of swelling at the lateral aspect of the right upper arm, which was of normal skin color, mobile, nontender without any dilated veins, or elevated local temperature since 5 years and was diagnosed as Medoja Granthi. She was treated with Apamarga Ksharasutra ligation locally along with Triphala Guggulu (250 mg), two tablets twice a day orally with lukewarm water for 10 days, and Yastimadhu Ghrita local application at the base for 12 days. RESULTS AND CONCLUSIONS: In this case study, the pedicle of the lipoma de-sloughed completely on the 10th day of Ksharasutra ligation. Thus, Apamarga Ksharasutra therapy showed promising outcomes in pedunculated lipoma treatment without any adverse effects. Randomized clinical trials on an appropriate sample size can help establish the clinical efficacy of the treatment procedure adopted in the current case study in the management of Medoja Granthi.
