Management of Infantile Blount's Disease With Molded Orthoses: A New Perspective

Abstract: Objective: This retrospective study evaluated treatment with individually contoured molded bracing at early stages of the disease. Methods: We evaluated the medical records of patients undergoing treatment of Blount's disease with molded orthoses for medial decompression between 2010 and 2014. The deformity angle (Drennan's metaphyseal-diaphyseal angle) and Langenskiöld classification were measured before and after treatment by a pediatric orthopedic surgeon with over 5 years of practice, blinded for the study… Show more

“…In some sources, the treatment duration of orthosis in ITV is suggested as a wider period ranging between 7 weeks and 2 years. Besides, the early period with the first signs of recovery is seen in the 3rd month 2 , 14 , 15 . Similarly, in this study, the effects of orthosis in the first period on children with ITV were investigated.…”

“…Lower spontaneous recovery rates and these patients seeking surgery in the absence of recovery are risks of waiting for spontaneous recovery. However, the success of orthosis in treatment is higher with the identification of eligible patients in the early stages 2 , 14 , 15 , 31 . For these reasons, a spontaneous recovery group was not established in our study, and the early outcomes of orthosis supported its effectiveness in treatment.…”

“…Surgical correction is required when patients fail bracing or present at an older age or with a more severe deformity [1,15,17]. Despite advances in the surgical treatment of Blount's disease, recurrence remains an issue, with reported rates as high as 88% [18][19][20].…”

“…Their recommendations were for children between 1½–2 ½ years of age, worn during weight-bearing to decrease compressive forces, and if not corrected after one year to proceed with surgery [ 48 ]. Authors report limited success with bracing due to multiple factors including compliance and obesity [ 1 , 49 , 50 ]. Bracing is not indicated in stages III and higher as it is ineffective and delays surgery.…”
