Management of listeriosis

Abstract: Determination of the MIC in vitro is often used as the basis for predicting the clinical efficacy of antibiotics. Listeriae are uniformly susceptible in vitro to most common antibiotics except cephalosporins and fosfomycin. However, the clinical outcome is poor. This is partially because listeriae are refractory to the bactericidal mechanisms of many antibiotics, especially to ampicillin-amoxicillin, which still is regarded as the drug of choice. A true synergism can be achieved by adding gentamicin. Another p… Show more

“…In case of an allergy to penicillin, cotrimoxazole is an alternative agent. Bactericidal effects could be documented with cotrimoxazole, aminoglycosides, teicoplanin and vancomycin [7]. The use of cephalosporins, quinolones, chloramphenicol, erythromycin and tetracycline is not recommended. The length of therapy depends on the type of listeriosis [1].…”

“…Ampicillin and penicillin G have extended in vitro bactericidal activity at concentrations attainable in the CSF [15]. Combination therapy with gentamicin, a bactericidal agent, showed synergic effects and is recommended in treating listeria CNS infections, endocarditis, and infections in immunocompromised patients, such as patients with SLE [11,16,17]. Trimethoprim-sulfamethoxazole, a bactericidal agent against L. monocytogenes that attains adequate levels in serum and CSF, is an eligible alternative and has elucidated clinical efficacy in treating listeria meningitis for patients allergic to penicillin [18,19].…”

“…Die Inzidenz für Listerioseerkrankungen ist in Deutschland mit 0,37 Erkrankungen pro 100 000 Einwohner nicht hoch, wobei es aber regionale Unterschiede gibt; so reichen die Werte von 0,16 gemeldeten Erkrankungen pro 100 000 Einwohner in Mecklenburg-Vorpommern bis 0,63 in Bremen [1]. Die Letalitätsraten betragen je nach Population und Verlaufsform 10-35% [1,15].…”
