Management of radial clubhand with gradual distraction followed by centralization

Abstract: Background:Treatment of radial clubhand has progressed over the years from no treatment to aggressive surgical correction. Various surgical methods of correction have been described; Centralization of the carpus over the distal end of the ulna has become the method of choice. Corrective casting prior to centralization is an easy and effective method of obtaining soft tissue stretching before any definitive procedure is undertaken. Moreover, it helps put the limb in a correct position. The outcome of deformity … Show more

“…The use of an axial pin to prevent recurrent radial tilt does prevent recurrence but also does not permit wrist motion [24]. As an alternative to centralization or radialization the Vilkki procedure developed in 1992 [25] performs a microvascular transplant of the second toe and metatarsal to support the radial side of the wrist.…”

“…Ограничение амплитуды движений в локтевом суставе достаточно часто встречается у пациентов с врожденной лучевой косорукостью. При наличии выраженной контрактуры в локтевом суставе удлинение локтевой кости не может быть проведено [29]. В среднем амплитуда движений в локтевом суставе составляет 99° [30].…”

“…However, this seems not to provide lasting correction, especially in severe forms [16,17]. The causes of recurrence of radial deviation found in the literature comprise residual retraction on the radial side of the wrist, premature ablation of the stabilization wire, and poor quality of transferred muscle [5,18].…”
