Management of Simple Hepatic Cyst

Abstract: Simple hepatic cysts are typically saccular, thin-walled masses with fluid-filled epithelial lined cavities that arise from aberrant bile duct cells during embryonic development. With the development of diagnostic modalities such as ultrasonography, computed tomography and magnetic resonance imaging, simple hepatic cysts are seen with relative frequency in daily clinical examination. US is the most useful and noninvasive tool for the diagnosis of simple hepatic cysts, and can generally differentiate simple hep… Show more

“…Because of the increased cyst size and increased mesenteric adipose tissue density around the cyst on CT and the septal structures within the cyst on US, we suspected cyst infection and performed cyst puncture, which revealed findings of a cystic infection, i.e., increased inflammatory cells and presence of pathogens, leading to the definitive diagnosis [11]. When a cyst with septal-like structures is seen on US, hydatid cysts must be differentiated [12], but hemorrhage can also cause septal-like structures due to intracystic blood clots [13]. In our case, the drainage fluid was reddish brown, suggesting that the septal-like structures were caused by hemorrhage in combination with infection.…”

“…However, it should be noted that attributing symptoms to simple cysts should be done with caution after ruling out all other possible causes. To manage simple hepatic cysts, it's important to correctly differentiate them from neoplasms and infections, and then choose a reliable treatment, including percutaneous aspiration, aspiration followed by sclerotherapy, or surgery [3]. Laparoscopic deroofing has become the preferred method due to its low recurrence rate, less postoperative complications, and exact efficacy [4][5][6][7][8][9][10].…”

“…Simple hepatic cysts (SHCs) are the most commonly diagnosed benign liver lesions, with a prevalence of 18 % in the general population undergoing abdominal computed tomography (CT) scanning for unrelated pathologies 1 . They are usually asymptomatic and detected incidentally, given the development and widespread use of diagnostic modalities 2 . However, larger lesions may present with clinical signs such as abdominal pain, epigastric fullness, early satiety or even jaundice.…”
