Patients with congestive myelopathy due to spinal dural arteriovenous fistula (SDAVF) typically present with progressive sensory and motor disturbance in association with sphincter dysfunction. Spinal MRI usually shows longitudinally extensive T2 signal change. Here, we report four patients with progressive myelopathy due to SDAVF who also presented with findings on cerebrospinal fluid (CSF) examination suggestive of an inflammatory aetiology. Such CSF findings in SDAVF are important to recognise, to avoid the erroneous diagnosis of an inflammatory myelitis and inappropriate treatment with immunosuppression. SDAVF can be difficult to detect and may require repeated investigation, with formal angiography as the gold standard.

Спинальные дуральные артериовенозные фистулы (спДАВФ)-наиболее частая сосудистая патология спинного мозга, обусловливающая сегментарное поражение спинного мозга за счет венозной ишемии. Функциональные исходы лечения пациентов с спДАВФ благоприятны, но вероятность улучшения варьирует от 25 до 100%, и такой диапазон затрудняет прогнозирование исхода. Цель исследования-выявить взаимосвязь между локализацией фистулы и клиническими ее проявлениями, оценить влияние длительности заболевания, степени неврологических нарушений на непосредственные и отдаленные результаты лечения на основании анализа публикаций и собственных данных. Материал и методы. В сентябре 2016 г. осуществлен поиск публикаций в базе данных PubMed по ключевым словам «spinal arteriovenous fistula», «treatment», «outcome». Отобраны публикации, содержащие информацию о возрасте пациента, локализации фистулы, длительности заболевания и оценке выраженности симптоматики по Aminoff-Logue scale до и после операции, а также не менее чем через 3 мес после вмешательства. В анализ, кроме того, включены данные о пациентах, оперированных в нашей клинике. В общей сложности анализ охватил 187 пациентов. Результаты. Наиболее частая локализация фистулы соответствовала уровню ThVI, ThVII и ThIX позвонков, бóльшая выраженность двигательных нарушений наблюдалась при локализации фистулы на уровне или ниже ThIX позвонка. Хирургическое разобщение фистулы приводит к улучшению функционального состояния больных, при этом пациенты в возрасте до 60 лет имеют лучшие шансы на восстановление нарушенных функций. Двигательные нарушения значимо регрессируют в раннем послеоперационном периоде у всех пациентов, но в отдаленном периоде наблюдается нарастание двигательных расстройств у пациентов с исходно лучшим функциональным состоянием.

The spinal dural arteriovenous fistula (SDAVF) is rare, presenting with progressive, insidious symptoms, and inducing spinal cord ischemia and myelopathy, resulting in severe neurological deficits. If physicians have accurate and enough information about vascular anatomy and hemodynamics, they achieve the good results though the surgery or endovascular embolization. However, when selective spinal angiography is unsuccessful due to neurological deficits, surgery and endovascular embolization might be failed because of inadequate information. We describe a patient with a history of vasospasm during spinal angiography, who was successfully treated by spinal stereotactic radiosurgery using Novalis system.
