Management of Zygomatic Fractures

Abstract: This is the largest reported survey on the repair of zygoma fractures. The response rate suggests dominance of OMFS in zygoma fracture care, an area pioneered by PS. Evolution of technique is also evident by predominance of ORIF with emphasis of multiple points of exposure, reduction, and fixation with rigid hardware.

“…However, in a retrospective study of 210 cases of ZMC fractures, 84% (n = 177) patients had ORIF, whilst 16% (n = 33) had closed reduction [4] . A survey answered by over 1600 ENT, OMFS and plastic surgeons that showed 81% would choose ORIF for ZMC fractures [15] . None of the ZMC fractures in our cohort were treated with closed reduction.…”

“…Some centres carry out orbital floor exploration in cases of primary diplopia or evidence of comminuted ZMC fractures only [17] . Interestingly, as demonstrated in a survey involving facial reconstructive surgeons, it was shown that 35% would carry out an orbital floor exploration routinely [15] . The proportion of patients in our cohort undergoing orbital floor repair was also lower when comparing to the literature.…”

“…The second most affected area was zygoma in the area of one-third of the facial half by the study of Baylan JM et al 20 This is due to the structure of zygoma which is more prominent than the other facial bones and has multiple articulations with other facial bones making it vulnerable to fractures on impact. 3,11 A totally different case happened in this study, where the highest prevalence of trauma was maxillary fractures (33.6%) followed by zygoma fractures (28.4%) and mandibula fractures (15.5%) respectively.…”

“…The absence of correct and immediate diagnosis of zygomatic bone fractures can lead to incorrect treatment with severe aesthetic and morphological implications, much more di cult to correct secondarily [5]. Regardless of the chosen therapeutic method, the treatment of zygomatic fractures is aimed at restoring orbital contour with the recovery of ocular globe functions, opening of the mouth within physiological limits, restoration of the sagittal, vertical and horizontal diameters of the midface by reduction of the fractured fragments in anatomical position and prevention of late complications [1][2][3][4][5][6].…”
