Managing periprosthetic joint infection—a qualitative analysis of nursing staffs’ experiences

Abstract: Background Periprosthetic joint infection represents a major complication in orthopaedics and trauma surgery. For an ideal management approach, it is important to understand the distinct challenges for all persons involved in the treatment. Therefore, it was aimed at investigating (1) the impact of periprosthetic joint infection (PJI) on the well-being of nursing staff to (2) identify challenges, which could be improved facilitating the management of PJI. “…Due to the frustration, some patients seemed to show insufficient compliance regarding the treatment. Also, in the field of PJI it was reported that non-compliance often resulted from the fact that patients were not aware of the severity of the infectious complication and thus, did not expect a long-time course of treatment ( Walter et al, 2022b ). To address this uncertainty among patients more in-depth information could be given to them and patient education programs could be launched.…”

“…Next, an abstraction was performed, meaning that the condensed units were labeled with a code by two of the researchers independently. After grouping the codes, data were categorized in themes as described previously ( Graneheim and Lundman, 2004 ; Svensson et al, 2020 ; Walter et al, 2022b ). Finally, themes were assigned to the categories, i.e., an interpretative meaning of related data.…”

“…6 For example, in 2022, Walter et al carried out a qualitative analysis of nursing staff's experiences in PJI management, highlighting that PJI has a high emotional burden for the patients, owing to the lack of social interaction contributing to it. 7 The difficulty of HI and PJI is mainly ascribed to difficulty in the timely correct diagnosis. The unmet medical and societal need is very well addressed in a review 8 written by several scientists belonging to AO Research Institute Davos, Switzerland, University Hospital of Basel, Switzerland, Ghent University, Belgium, University Hospitals Leuven, Belgium, Trauma Centre, Murnau, Germany, University of Rochester Medical Center, New York, USA, MC, University of Amsterdam, The Netherlands, Virginia Commonwealth University, Virginia, USA that reported, “ A considerable number of infections are ‘culture-negative’ despite being clinically apparent.…”

“…The aftermath of FRIs encompasses a spectrum of challenges, including immobility, potential fear of amputations, extended hospital stays, multiple surgeries, and the socioeconomic implications of income loss. 8 , 9 The financial burden is substantial, with direct healthcare costs estimated to be six to eight times higher than in noninfected cases, largely attributed to extended hospitalization. 10 , 11 Recent consensus regarding the definition of FRI offers clarity, yet effective management remains intricate, necessitating interdisciplinary collaboration and comprehensive guidelines.…”
