Mechanisms of Linezolid Resistance among Staphylococci in a Tertiary Hospital

Abstract: bThe mechanisms of linezolid resistance among 86 staphylococcal isolates from two intensive care units were investigated. The most frequent was the G2576T mutation in the 23S rRNA (82%). The cfr gene was found in 17% of the isolates, seven S. aureus and eight S. epidermidis isolates. Four of the S. epidermidis isolates had the G2576T mutation and the cfr gene. In four S. haemolyticus isolates, the mechanism could not be identified.

“…Linezolid belongs to the oxazolidone class of antibiotics and is a protein synthesis inhibitor (Kloss et al, 1999 ) with bacteriostatic action. The resistance gene responsible for linezolid resistance was identified as cfr gene, encoding a methyltransferase that modifies the 23S rRNA site of the 50S ribosomal subunit, preventing linezolid to bind to it (Toh et al, 2007 ; Quiles-Melero et al, 2012 ). Work on these organisms reported the presence of cfr gene in chromosomes of clinical human MRSA isolates as being responsible for mediating resistance in MRSA (Morales et al, 2010 ).…”

“…Linezolid binds to the 50S ribosomal subunit of the 23S rRNA molecule and inhibits protein synthesis. Cfr gene encodes a methyltransferase that modifies the 23S rRNA site of the 50S ribosomal subunit and prevents linezolid from binding to it [14]. Q/D is composed of two streptogramins (70% dalfopristin (streptogramin A) and 30% quinupristin (streptogramin B)), which was approved in 1999 as a treatment option for VRE and MRSA infections.…”

“…The safety and tolerability of linezolid favour its use in prolonged treatments, but it should be taken into account that continuous exposures during long periods increase the chance of selecting isolates with higher mutation dosages and higher linezolid MICs. [5][6][7][8] …”
