Medikationsprobleme bei Entlassung aus dem Krankenhaus und Implikationen für die trans kulturelle Versorgung

Abstract: ZusammenfassungGegenstand: Entlassungen aus dem Krankenhaus bergen Risiken für die Kontinuität und Sicherheit medikamentöser Versorgung. Insbesondere im Kontext transkultureller Medizin stellt diese Transitionssituation eine besondere Herausforderung dar. Methoden: Vier Fokusgruppeninterviews wurden mit insgesamt 42 Pflegetrainern geführt und mit qualitativer Inhaltsanalyse ausgewertet. Ergebnisse: In der Entlassungssituation ist auf professioneller Ebene vielfältiger Fehlumgang mit Medikation zu verzeichnen. … Show more
