Bio-Psycho-Socio-Spirito-Cultural Factors of Burnout: A Systematic Narrative Review of the Literature

Background: Burnout is a widespread, multifactorial, and mainly psychological phenomenon. The pathogenesis of burnout is commonly described within the bio-psycho-social model of health and disease. Recent literature suggests that the phenomenon of burnout may be broader so that the three dimensions might not reflect the multifaceted and complex nature of the syndrome. Consequently, this review aims to identify the diversity of factors related to burnout, to define overarching categories based on these, and to clarify whether the bio-psycho-social model adequately describes the pathogenesis of burnout—holistically and sufficiently.Method: Five online databases (PubMed, PubPsych, PsychARTICLES, Psychology and Behavioral Sciences Collection, and Google Scholar) were systematically searched using defined search terms to identify relevant studies. The publication date was set between January 1981 and November 2020. Based on the selected literature, we identified factors related to burnout. We aggregated these factors into a comprehensible list and assigned them to overarching categories. Then, we assigned the factors to the dimensions of an extended model of health and disease.Results: We identified a total of 40 burnout-related factors and 10 overarching categories. Our results show that in addition to biological, psychological, and socio-environmental factors, various factors that can be assigned to a spiritual and work cultural dimension also play an important role in the onset of burnout.Conclusion: An extended bio-psycho-socio-spirito-cultural model is necessary to describe the pathogenesis of burnout. Therefore, future studies should also focus on spiritual and work cultural factors when investigating burnout. Furthermore, these factors should not be neglected in future developments of diagnosis, treatment, and prevention options.

Aiming to identify the mechanisms generating positive clinical supervision outcomes in child protection, this narrative review provides an in-depth analysis of the theories underpinning clinical supervision in the latest child protection literature. The conceptual analysis of 28 peer-reviewed journal articles highlighted the presence of the psychodynamic, managerialist, critical, behavioral, systemic, humanistic, and eclectic theoretical perspectives. Implicit theoretical eclecticism permeated most of the publications examined. This eclecticism resulted in confusing child protection practices as different theories require different practice techniques and result in different practice outcomes. The study found that half of the publications exclusively adopted the critical and managerialist theoretical perspectives that undervalue the impact of internal factors in the behaviors of families and practitioners. Despite the fact that all the publications acknowledged the centrality of emotions in supervision, only the psychodynamic theoretical perspective elaborated on the precise process through which emotions are conceptualized in clinical supervision. Because most of the publications neither identified the operationalization process nor evaluated any clinical supervision outcomes, questions arise about the theoretical robustness and essentially the effectiveness of child protection practice itself. Therefore, a need emerges for case studies to explore the process through which theory-bound clinical supervision practices generate effective child protection outcomes.

The way social workers discursively construct ‘service user’ identities in everyday interactions (interviews, conversations and text) can affect quality of relationships and practice outcomes. Even though research has focused on the construction of ‘service user’ identities by professionals and service users, little has been done to explore such discursive formulations by pre-qualifying social work students. This is especially relevant, given the strengthening of the ‘expert by experience’ identity in social work education. This paper seeks to make visible mechanisms of student identity constructions as to ‘who a service user is’, and implications for practice through the examination of student written work pre- and post- a module focussing on lived experience. A critical discursive psychology approach was followed, recognising the interplay between localised professional encounters and wider contexts of power relations. The findings show a shift in the ‘service user’ identities employed by the students mainly based on individualistic discourses and deserving/undeserving themes (substance misuse the result of vulnerability, rather than selfishness, domestic abuse narratives denoting resilience rather than victimhood). The effect to practice showed shifts between the reflective, expert, person-centred and critical/radical practitioner, mainly stressing the need for professional growth at an individual level, with less emphasis on addressing social inequality. The paper argues that predominantly individualistic discourses can perpetuate de-politicised or oppressive categorisations of ‘service users’ and calls for further critical engagement with the discursive micro-practises enacted and developed in the social work classroom, if we are to unveil and challenge narrow, or stigmatising categorisations early on.
