Metastatic prostate cancer presenting as an asymptomatic neck mass

Abstract: Prostate cancer often metastasizes to the regional lymph nodes, but metastases to distant supradiaphragmatic lymph nodes are uncommon. Rare case reports describe cervical lymph node metastases as the first clinical manifestation of prostate cancer, but only in the setting of widely disseminated disease. We present the unusual case of an 84-y-old male with a known history of prostate cancer and recurrent disease limited to the left supraclavicular lymph nodes.

“…However, uncommon sites of prostate cancer metastasis have been reported in the literature, including orbit/skull base, liver, intracranial, ocular, renal, adrenal and neck involvement [4][5][6][7][8]. The high incidence of prostate cancer and the fact that unusual metastasis can be the presenting feature of malignancy or the only sign of distant spread make the awareness of atypical metastatic sites important for clinical practice.…”

“…The biopsy specimen showed adenocarcinoma. Immunostaining for cytokeratins 8,18,19 (CK 8,18,19) and PSA was positive, while immunostaining for thyroid transcriptional factor-1 (TTF-1), leukocyte common antigen (LCA) and cytokeratin 20 (CK20) proved to be negative. The immunostaining results substantiated the prostate origin (Fig.…”

“…In a series consisting of 753 post mortem examinations, regional lymph node metastases in 63% of the cases were shown, and while a portion of these involved pelvic and paraaortic lymph nodes another portion involved only paraaortic region [3]. Distant metastases to supra diaphragmatic lymph nodes are rare, and occur most frequently in the left supraclavicular region in a limited number of cases; however, this is seen only in patients with widespread metastases [4][5][6][7].…”

“…Diffuse lymphadenopathy from prostate cancer is rare in the PSA era. Distant lymphatic spread beyond the retroperitoneum to Virchow's (supraclavicular) node is particularly rare 2, 3, 4. This distribution is more commonly suggestive of lymphoproliferative disorders.…”
