Microstent-Assisted Coiling for Wide-Necked Intracranial Aneurysms

Abstract: In the past years, the concepts of treatment of intracranial aneurysms have changed and evolved. New technologies have contributed to expand the options for endovascular management of both ruptured and unruptured intracranial aneurysm.1-4 The A B S T R A C T: Objective:To describe the results, technical feasibility, efficacy and challenges encountered in our preliminary experience using a self-expandable microstent, optimized for intracranial use, as an adjunct in the endovascular treatment of wide-necked aneu… Show more

“…There was no short-term or long-term clinical or radiographic outcome data available for these 2 studies. 21,22 In a recent study, 23 symptomatic thromboembolic events were observed in 3/17, with 2 being transient and 1 presenting permanent deficit, which appeared to have a higher incidence than previous studies. 21,22 There was no mortality and a favorable clinical outcome (modified Rankin scale score 0-2) was observed in 88.2% of the patients.…”

“…21,22 There was no mortality and a favorable clinical outcome (modified Rankin scale score 0-2) was observed in 88.2% of the patients. 23 Lylyk et al studied 46 patients with 48 intracranial aneurysms and reported 6 thromboembolic events (12.5%) and 1 procedure-related death due to arterial rupture (2.1%). 24 We encountered relatively less clinical and technical challenges compared to the studies published in previous reports 21,22 and experienced no mortality in our series ( Table 1).…”

“…Self-expandable nitinol stents dedicated to neurovascular systems have been widely used, and the first clinical results have been reported. 4,12 Note, however, that there have been no published mid-or long-term results for self-expandable stent-assisted coil occlusion of large or giant aneurysms.…”

“…The fundamental technique involving the Hyperglide balloon and the Rebar 14 microcatheter (both products of MicroTherapeutics, Inc.) to embolize aneurysms with the Onyx liquid polymer has been previously described. 4,5,7 In this paper we describe some technical variations in the use of Onyx, combining it with intrasaccular coils. All patients were treated while in a state of general anesthesia and full systemic anticoagulation with heparin.…”
