Missed opportunities to prevent mother‐to‐child transmission of HIV in Italy

Abstract: ObjectivesVertical transmission of HIV can be effectively controlled through antenatal screening, antiretroviral treatment and the services provided during and after childbirth for mother and newborn. In Italy, the National Health Service guarantees universal access to prenatal care for all women, including women with HIV infection. Despite this, children are diagnosed with HIV infection every year. The aim of the study was to identify missed opportunities for prevention of mother-to-child transmission of HIV.… Show more

“…Therefore, it seems to be clear that many HIV related events probably wouldn't have occurred if diagnosis and therapy were more precocious and if adherence to the treatment was greater. As exposed in a recent study by Di Biagio et al (58), numerous efforts, both medical and organisational, are still needed to improve the survival and quality of life of children with perinatal HIV-1 infection. For this purpose, it is essential to ensure early diagnoses, to start treatment early and to improve adherence to therapy in the paediatric population.…”

“…An additional possibility for not having been diagnosed with HIV prior to conception is that women did not actually contract HIV until the time of conception or early during their pregnancy. In a recent Italian study, 15% of women with perinatal transmission to child tested negative at the start of pregnancy and developed an acute infection during pregnancy [ 28 ]. Future studies should explore whether women diagnosed during the third trimester had previously tested negative.…”

“…On the contrary, in the past, due to high HIV-related mortality in childhood [ 38 ], many PHIVs did not have the chance to reach adulthood and, therefore, there are no historical data on the impact of long-term treatment and on how it is best to treat them over time. This work might have a bearing on potentially greater burden HIV settings, such as Africa, and also in industrialized countries, where mother to child transmission has not been eliminated yet [ 39 ].…”
