Mixed chimerism through donor bone marrow transplantation

Abstract: This review will address the recent advances in murine models as well as findings from the first clinical trials for the induction of tolerance through mixed chimerism. Both the potential for more widespread clinical application and the remaining hurdles and challenges of this tolerance approach will be discussed.

“…Treg cells manufactured with certain homing properties represent a suitable therapy with potential applications in inflammatory bowel disease [99–101], allergy to food antigens [102], and inducing tolerance in bone marrow transplantation as well as solid organ transplantation [102,103]. Homing molecules to the intestine [104] and bone marrow [105] have been well characterized.…”

“…The term mixed chimerism has been subjected to a widely encompassing definition, ranging from donor hematopoietic stem cell engraftment with stable long-term multilineage contribution, to regimens using donor bone marrow infusion to achieve transient mixed chimerism that may or may not be followed by a state of microchimerism. These various states of chimerism and the relative role they play in the induction and maintenance of tolerance of transplanted tissues have been demonstrated to vary across models and target organs [8]. …”

“…Among all these, strategies based on the use of co-stimulation blockade are the most potent at inducing mixed chimerism (6). However, there are few reports on protocols for promoting stable intestinal allograft acceptance.…”

“…Establishment of mixed chimerism through transplantation of donor bone marrow into recipients is one of the most promising strategies for inducing transplantation tolerance (5). Most of the chimerism-inducing protocols require the use of co-stimulation blockade agents (6). Recent results from experimental mouse studies based on co-stimulation blockade induction of stable mixed chimerism are encouraging.…”
