Modified Uvulopalatopharyngoplasty and Coblation Channeling of the Tongue for Obstructive Sleep Apnea: A Multi-Centre Australian Trial

Abstract: Study Objectives: To investigate the surgical outcomes and effi cacy of modifi ed uvulopalatopharyngoplasty (mod UPPP) and Coblation channelling of the tongue (CCT) as a treatment for obstructive sleep apnea (OSA). Methods: Adult patients with simple snoring or obstructive sleep apnea were treated with combined modifi ed UPPP, bilateral tonsillectomy, and CCT (N = 48). Full polysomnography was performed preoperatively and 3 months postoperatively. Postoperative clinical assessment, sleep questionnaires, and pa… Show more

“…5 In addition, a recent publication supports a lesser operation (modified uvulopalatopharyngoplasty and coblation tongue channelling) when clinical assessment dictates. 22 The procedures utilised in the 22 airway reconstruction operations reported here (for 17 patients) have been described elsewhere. These included a modified Robinson type uvulopalatopharyngoplasty, 22 transpalatal advancement, 23 coblation channelling of the tongue, 24 endoscopic lingual tonsillectomy or lingual tonsil reduction, submucosal lingualplasty, 25 and a new procedure referred to as a coblation-assisted Lewis and MacKay operation ('CobLAMO') 26 .…”

“…A transcervical technique to access the retrolingual space and perform radiofrequency ablation of hypertrophic lingual tonsils under sedation and local anesthesia has been reported and its effectiveness remains to be established 57. Other newly developed surgical approaches include modified uvulopalatopharyngoplasty and coblation channeling of the tongue, a coblation-assisted Lewis and MacKay operation, and transpalatal advancement 58,59. An investigational technique, typically used within a staged surgical approach, is transoral robotic surgery for tongue base reduction 60…”
